What do I need to do before I’m ready for a Care Management Plan?

As a quick recap:

Your management plan is critical to your agreement and you have to do it in conjunction your Package Provider, so what should be taken into account in developing your plan?

Your Package Provider should take into account any support you already have in place. This might include carers, family members, local community support, etc.  A Home Care Package is intended to meet needs that are not already being met by these other supports. 

Your provider will already have some information about you from your ACAT/ ACAS assessment and this can help guide the discussion when it comes to your goals, preferences and needs. 

Working through this will help you make informed choices about what you’d like to receive. 

Your care plan will broadly cover:

  • Your goals (what do you want to achieve through the package?)
  • Care needs  
  • Level of involvement and control you will have in managing and coordinating your Home Care Package
  • The exact care and services to be provided and who will provide them
  • When the services are to be delivered including the frequency of services and days/ times when regular services are expected to be provided
  • Case management arrangements including how to manage monitoring and informal reviews 
  • Frequency of formal reassessments
